Helen & Douglas House Events Survey 2023

*Terms and conditions for prize draw: 1. The free draw is open to all respondents who complete the survey. Staff members of Helen & Douglas House and any other organisation connected with the survey are not eligible to enter the free draw. 2. Entry to the free draw is by completing the survey. 3. Only one entry per person. Multiple entries will be disqualified. 4. There is 1 prize: £50 Amazon gift card 5. The free draw closes at 11.59pm on 1st February 2023 (with winners drawn before 8th February 2023). 6. The winner will be notified by email by 8th February 2023 to the email address they specified when they completed the survey. If the winner does not claim the prize within 30 days of being notified, then they will forfeit the prize and a re-draw will take place. To claim the prize the winner must follow the instructions in the email notifying then that they are the winner. The Winner will receive their prize no later than 30 days after they claim the prize. 7. The winner is responsible for paying any income tax (if any) due on the prize won. 8. There is no cash alternative to the prize. 9. No responsibility will be accepted for incomplete or lost entries or entries not received. 10. In the event of any dispute, Helen & Douglas House’s decision is final and no correspondence will be entered into. 11. The name of the winner will be available after 8 February 2023 by emailing events@helenanddouglas.org.uk 12. The free draw will be governed by English law and entrants to the free draw submit to the jurisdiction of the English courts. 13.
