Youth and Transition
We offer support for young people who are transitioning from child services to adult servicesAs your child gets older, the move from familiar children’s services to new adult services can feel very daunting.
Transition is not a single event, but a gradual process, ideally beginning at around the age of 14 years to tie in with the Year 9 school annual review. A good transition should build on your young person’s strengths and wherever possible work with them to plan for how they can meet their aspirations in life.
Ideally it involves your child and parents/carers to support you to understand the changes that will happen in the next few years. It should highlight you, in your own right, and as carers as your child reaches the legal age of adulthood. The legal age of becoming an adult is 18 in the UK.

How we can support you
It’s important that your child is involved in all the discussions about their future and feel able to speak up when they don’t understand something or feel that they are not being properly listened to. This is not always easy. Some professionals may sound as if they know everything. There may be times when your child feels as if they need some support in order to get their views heard. Someone who acts on their behalf is known as an advocate and they can play an important role in making sure their move to adult services goes smoothly.
One to one sessions
We understand having someone to listen to you, advocate for you and do special activities with is important. We offer one to one sessions for young people aged 13+ who have capacity and who would benefit from engaging with another person. Sessions are flexible and can take place in school, at home or at the hospice.
Youth group
We offer young people the chance to get together with their peers in a youth group called Network Saturdays. From weekend sleepovers, to river trips, ice cream trips and themed days, this is a chance for the young people to socialise, build confidence and find lasting friendships.

There are so many aspects of social care that can feel daunting, but we’re here to offer our support on everything from laws to finances.
Education
Whatever road your child decides to take with their education, we can provide support and advocacy on next steps and how to navigate further education.
Healthcare
We can give advice on managing your child’s healthcare needs as they transition to adult services. From GP annual health checks, to transition to an adult hospice setting.
Work/life balance
Finding the right balance with a long-term health condition can be tough. We want young people to have the knowledge to be able to face potential challenges and changes in their lives with confidence.
